FERTILITY PRESERVATION GRANT PROGRAM

We are dedicated to helping young men and women with a cancer diagnosis who require financial assistance to preserve their fertility. While we aim to help as many individuals as possible, please note submitting an application does not guarantee financial assistance. All requests are carefully reviewed by the JC Memorial Foundation Board of Directors and funding decisions are made based on available resources and the eligibility of the applicant.

  • Individuals must be referred by a hospital social worker or hospital medical staff.

  • Amount received will be determined on an individual basis, determined by each individual’s financial need, insurance coverage, household income and the funds we have available.

  • Any payment awarded will be made directly to the fertility clinic. No payments are made to the patient.

Additional Fertility Service Financial Assistance Resources

  • Heart Beat Program provides select fertility medications at no cost for eligible patients who have received a diagnosis of cancer and have received no chemotherapy within the previous 12 months.

  • International Council on Infertility Information Dissemination provides an IVF scholarship to help those who cannot afford IVF treatments. The IVF procedure is donated by reproductive specialists. In many cases, you will have to travel for treatment.

  • LIVESTRONG Fertility provides financial assistance for fertility treatment for those diagnosed with cancer. 

  • Reunite Oncofertility helps to reduce the costs for fertility preservation by offering a discount of up to 100% on select medications

  • Team Maggie for a Cure provides grants to uninsured/underinsured teen and young addults (ages 15-37) who are receiving treatment for cancer.

  • Verna's Purse provides financial assistance to patients faced with economic challenges in efforts to preserve their fertility.

  • Worth the Wait provides financial support for fertility treatments, adoption and surrogacy to young adult cancer survivors pursuing parenthood.
